Health Office Notes 2018



This is to inform you of some important changes in Education Law effective July 1, 2018
 
Beginning July 1, 2018 all NYS public school students must have a health examination when they enter as a new entrant and in Pre-K or Kindergarten and in grades 1, 3, 5, 7, 9, and 11.  A dental certificate which states your child has been seen by a dentist or dental hygienist is also asked for at the same time.
 
A copy of the health examination must be provided to the school within 30 days from when your child first starts school, and when your child starts grade K, 1, 3, 5, 7, 9, 11.  If a copy is not given to the school within 30 days, the school will contact you.
 
If your child has an appointment for an exam during the school year that is after the first 30 days of school, please notify the Health Office with the date.
 
AND
These examinations must be completed on the approved NYSED Student Health Examination Form: ReqNYSSchoolHealthExamForm.pdf
 
We understand that your medical provider’s office may not yet be aware of the change, so if you have already had a physical examination completed for the 2018-19 school year on a different form, the school may accept the physical on that form.
 
In 2019-2020, ONLY the approved form will be accepted.
 
Also, there have been changes to the vision, hearing, and scoliosis screenings.  In 2018-19 school year, screenings will be done as follows:
  • Vision screening will now include distance acuity and near vision acuity and will be done in grades PK or K, 1, 3, 5, 7, 11.
  • Hearing screening will be done in grades PK or K, 1, 3, 5, 7, 11.
  • Scoliosis screenings will be done for girls in grades 5 and 7, and for boys in grade 9.
If any of these screenings are done by your child’s health care provider and documented on their Health Examination Form, they will not be repeated in school.
Any abnormal screening results will be mailed to you.

Immunization requirements for the 2018-19 school year:
Students entering PK or K: immunizationrequirements2018-19.pdf
Students entering into 6th grade will need a Tdap & 2nd Varicella (chicken pox) vaccine
Students entering into 7th grade will need the first of two meningococcal vaccines
Students entering into 12th grade will need the 2nd meningococcal vaccine
 
If your child receives these immunizations at their health care provider, you must provide proof of immunization to the school before the start of the school year.
 
If your child does not receive these immunizations before the start of school, they may be excluded from school until they meet immunization requirements.
